 Emergency Action Plan (EAP) Development

The following information is being provided for State Agency Building Coordinators for developing or revising an existing Emergency Action Plan for their building.

Building EAP Template - General template for development of building specific emergency action plan.

Building EAP Development Guidance Document - Assists in obtaining the necessary information to complete the Building EAP template.

The guidance document also contains a Floor Diagram Check Sheet that identifies key emergency items that are to be identified on the posted floor diagrams for the general population in the building, and items that the Building Coordinator should be aware of as part of the overall EAP.

Floor Wardens: They are a key component in the development and implementation of your building's EAP.   The following templates will assist in selecting and training floor wardens:

Floor Warden Assignment Form - Used to identify your building's floor wardens. One Floor Warden should be assigned for each floor and responsible for no more than 20 employees. Additional searchers/sweepers can be assigned to assist Floor Wardens. 

Floor Warden Training Curriculum - Identifies areas to be covered in Floor Warden and Employee training.

Floor Warden Training Power Point Template - Used in the training and orientation of Floor Wardens.

Floor Warden Checklist for Employee EAP Orientation - Checklist for Floor Wardens to use when orienting employees on the building's EAP.

Floor Warden Training - Contact State Risk Management regarding training for new Floor Wardens.

Additional Training Resources:

Suspicious Mail and Packages - Available to all State Employees by logging into YES. Register by Category: Emergency Planning, Course: ERM100SP  for CBT training.  Contact  State Risk Management for training available for non-State Employees such as  volunteers and contracted personnel.

AED/CPR Training - Department of Health Services website for training and orientation on AED and CPR usage.

The following may be additional items that should be included in the building specific EAP:

Cafeterias - EAP procedures for a building containing a cafeteria.

Critical Operation Shutdown Procedures - Operations that may need to be shutdown during an emergency.

Employees Needing Assistance Form - Identifies individuals who may need assistance during an evacuation.

Panic Alarm Testing Procedures - Where applicable, testing procedures to ensure proper operation.

Communicator NxT - Go to the ADEMA website or contact the Communicator NxT Coordinator at ADEMA for additional information.

Evacuation Drills:

Agency occupied buildings should coordinate drills through their building coordinator, building owner  or property management company.

Floor Warden Pre-Drill Meeting Agenda - Meeting agenda of topics to cover prior to scheduled drills.

Evacuation Evaluation Form  - Complete following any evacuation.

Emergency Flip Chart - Version 1.3  and  Area Evacuated Door Hangers

Alternate Assembly Areas - Contact State Risk Management for alternative areas to assemble in the event weather or other issues require evacuees to be re-located during an emergency.
